Pursuant to __________, we are sending this courtesy notification regarding your child's school attendance.
As of today’s date, our records show that your child has accumulated at least 30 total hours of missed school so far this school year. This total includes both excused and unexcused absences, but excludes any time missed due to documented medical appointments or conditions.
To ensure absences are properly excused, a written note from a parent/guardian or a doctor must be submitted to the school office for each occurrence.
Please be advised that if your child’s attendance continues to decline, additional steps and interventions will be required as mandated by the State of Ohio and the [County Name] County Juvenile Court. These thresholds are:
