Medical Status Letter

Medical Status Attendance Notification


Dear Parent/Guardian of <<student_name>>,


Pursuant to __________, I am sending you this notification to inform you that <<student_name>> has now met the threshold for excessive absences. As of today’s date, our records indicate that your child has missed <<current_hours>> total hours of school this year.


As a result, <<student_name>> has been placed on medical status for the remainder of the school year. From this point forward, all absences must be verified by a signed note from a licensed medical provider. This documentation must be submitted to the school office within 48 hours of your child’s return to school, or the absence will be considered unexcused.

Once a student is classified as Excessively Absent, written notes from home will no longer be accepted, and the final determination of whether an absence is excused or unexcused will be made by school administration.


According to Ohio law, a student is considered Excessively Absent if they are absent from school, without a valid excuse, for:

  • 38 or more hours in a single school month (approximately 7 days for elementary students; 6 for middle/high school), or

  • 65 or more hours in a school year (approximately 12 days for elementary students; 10.5 for middle/high school).

Please remember that under Ohio law, parents are primarily responsible for ensuring regular school attendance, and school districts are required to enforce these attendance laws and implement intervention strategies when thresholds are reached.
